a. Go to your calculator.

b. Enter the amount in the box indicated.
c. Show or give a copy to your patient.

GET THEM TO SIGN THE CONSENT FORM

a. In your portal, click on Consent.

b. Click on Download to print.
c. Print a copy and have your patient sign.
d. Upload the signed copy to the query.

FILL IN THE REQUEST FORM

a. Fill in the requested information.

b. Enter your patient's e-mail address to ensure faster processing.
c. Download the treatment plan and your patient's signed consent form.
d. Submit the request.

TRACK YOUR REQUEST IN THE PORTAL

a. Find the contact details of the person responsible for your request in the For more information section.
b. Consult your patient's key information in the Patient section.
c. Access contract details and disbursement date in The financing contract section.
